Components of insulin resistance and chronic hyperglycemia as cardiovascular risk factors Several prospective epidemiological studies have shown that hyperinsulinemia predicts the risk of CHD in non-diabetic subjects 127, 270, 289, ; . Some crosssectional studies have shown that type 2 diabetes patients with atherosclerotic vascular disease AVD ; have higher concentrations of fasting plasma insulin or Cpeptide or higher postglucose plasma insulin concentrations than those without AVD 419, 431, 446 ; . In the UKPDS 475 ; fasting serum insulin concentrations were positively correlated with the risk of MI but this correlation became nonsignificant after adjustment for other risk factors. In contrast to these observations, in a 10-year follow-up of the "borderline" diabetic group of the Bedford Study, a low 2-h post-glucose plasma insulin was associated with an increased risk of CHD events 235 ; . Lehto S et al. 288 ; have recently shown in a 7-year prospective study that the predictive value of hyperinsulinemia with respect to death from CHD was independent of conventional cardiovascular risk factors but not of risk factors clustering with hyperinsulinemia. By applying factor analysis and principal component analysis it was shown that the "hyperinsulinemia cluster" a factor having high positive loading for BMI, triglycerides and insulin; and high negative loading for HDL cholesterol ; was predictive of death from CHD in type 2 diabetic patients. Kaukua et al. got similar results 247 ; . Other studies have also emphasized the association of hyperinsulinemia with several cardiovascular risk factors 194 ; . These data suggest that causes of the insulin resistance syndrome, such as obesity and consequences of insulin resistance such as hypertriglyceridemia and a low HDL.
The CD4 count is a quantitative measure of the status of the cell-mediated immune system. Normal CD4 cell counts are in the range of 500 to 1, 300 cells mm3. As disease progresses, CD4 cell counts decline. It is therefore desirable to check the CD4 counts at baseline, and periodically thereafter. Counts of less than 500 cells mm3 are atypical in HIV -uninfected persons. Identifiable causes of variation include time of day, short-term changes in health such as intercurrent illness, and experience of the clinical laboratory performing the test. Due to the diurnal variation in CD4 counts, it is recommended to draw blood at the same time of day for all estimations. The CD4 cell count suggests the state of a patient's immunologic function at that point in time. It is useful for considering both initiation of antiretroviral therapy as well as prophylaxis for opportunistic infections. The trend rate of fall ; in CD4 counts may be helpful in determining when to initiate antiretroviral therapy. At counts between 500 and 350 cells mm3, it is uncommon to find clinical manifestations of cell-mediated immune dysfunction. At counts below 350 cells mm3, thrush, zoster, recurrent dermatoses and other mucosal infections may signify impaired immune function, and patients with counts below 200 cells mm3 are at increased risk for life-threatening opportunistic infections. In this context, it is important to note that in India, TB has been reported at higher CD4 counts. The CD4 cell count alone does not always reflect the clinical status of an HIVinfected individual, in that untreated individuals with similar CD4 cell counts can have very different functional status, frequency of opportunistic infections and constitutional symptoms and signs. Table 7 lists complications of HIV infection based on total CD4 cell count that have been generally reported. However, a particular opportunistic infection may occur at a slightly earlier or later stage of the disease than mentioned in the table. Specimens for CD4 count estimation should be processed w ithin 18-24 hours, and ideally as soon as possible. Physiologic variations have been reported, especially at higher counts and combivent.

Three TMLT consultants reviewed this case. Their collective opinions were somewhat critical of the physician's choice of antibiotic. It was noted that during the incision and drainage, there were no bone samples submitted to pathology to seek a definitive diagnosis of osteomyelitis and obtain culture and sensitivity data. This lack of information left the defendant's indications for the use of a potentially toxic antibiotic in question. The patient was tested on multiple occasions after the date of loss with a demonstrated level of zero vestibular function, indicating a permanent injury in this relatively young plaintiff. In addition, the amount of Gentamycin prescribed by the defendant exceeded the PDR black box warnings that carry a recommended dosing schedule not to exceed 7 mg kg in patients with what is to be considered a lifethreatening condition. The dosage for lesser conditions is recommended not to exceed 5 mg kg. This patient's dosing schedule included amounts that were as high as 8 mg kg. One expert noted that the defendant's final order to increase the Gentamycin level was entered after the patient's lab reports indicated that she had achieved a therapeutic level of the drug and dulcolax.
Haemodynamic effects of thiazides Thiazide diuretics do not lower blood pressure in normotensive individuals, but are effective in hypertensive subjects.37 At first sight, the blood pressure lowering action of thiazides seems obvious; diuretics will cause a reduction in plasma volume, diminish venous return and depress cardiac output and blood pressure. This mechanism does seem sufficient to explain most or all of the reduction in blood pressure seen in hypertensive subjects following acute administration of thiazides, 38-42 and re-expansion of plasma volume by infusion of dextran, with or without sodium, restores blood pressure to pre-treatment levels.39, 40 In association with the fall in cardiac output following acute administration of thiazide, there is a rise in total peripheral vascular resistance TPR ; that is mediated largely by the sympathetic nervous system SNS ; 43 and activation of the reninangiotensin-aldosterone system.44, 45 Activation of the SNS may persist after chronic use, 43 but variability in reflex responses may contribute to differences in blood pressure lowering, at least in the short term. In the longer term, there is substantial evidence to suggest that effects on plasma volume and cardiac output cannot explain the hypotensive action of thiazides.There is a poor correlation between the dose of thiazide required for diuresis and blood pressure lowering46 and plasma levels of thiazide do not predict blood pressure lowering.47 Loop diuretics, such as frusemide, are more effective diuretics but less effective antihypertensive.
