Objectives: To assess the efficacy and safety of 1% ciclopirox in the treatment of SD of the scalp. Design Methods: Subjects with moderate SD of the scalp, meeting inclusion exclusion criteria, were enrolled. After a 2-week run-in phase with a cosmetic shampoo 2x weekly, they were randomized 2: 1 ; into 3 treatment groups to receive active drug 1x or 2x weekly, or shampoo vehicle. Subjects used ~5 ml per application or 10 ml if hair was shoulder-length. After 4 weeks of treatment, investigators evaluated global status 6-point scale from none to severe ; and change from baseline. Response rates at individual endpoint score of 0, or 1 baseline was 3 ; for the 1x- and 2x-per-week groups were compared sequentially with the response rates for vehicle by the Bonferroni-Holm procedure. The one-year study reported Hytrin to be approximately twice as effective as placebo. This study also demonstrated the drug's efficacy in severely symptomatic patients. This study suggests alpha-blockers for the treatment of severe BPH, additional clinical trials extending beyond one year and comparing other alpha-blockers would be helpful. Another valid question regarding efficacy refers to a possible interdependence between BPH therapies. In the model, each therapy was independent of previous therapies. One could speculate a specific therapy used before another might modify the succeeding therapy's efficacy rate or BPH complication rates; however, this is not fully substantiated. Results of the sensitivity analysis indicate this may not be a significant issue. While current literature supports an acceptable safety profile of finasteride, its true clinical efficacy appears less conclusive, with the suggestion that finasteride is more efficacious in men with larger prostates.39, 40.
